[Help] Repeating PLOT Instances to achieved 3 shuriken stars

The point is HOW TO OBTAIN 3 SHURIKEN(3 STARS ) PLOT INSTANCE, I repeated(Completed) each Plot instances numerous times, still the same result ONLY 2 SHURIKENS ( still no LUCK obtaining 3 SHURIKENS). What is required to obtain 3 SHURIKENS in completing PLOT INSTANCES?
