I was doing Hard Pain TI for some time and yes it does drop that sword, got it few times. It also seems to have higher drop rate for gold five nature stones than any of Brothers TI, while normal Brothers drop red stones more often.

Yes, but I think the drop rates are super messed up. I don't think I've even seen one to "miss" on my main account. But on my alt account I have 4 of them...
