Please submit a ticket through Customer Service Ticketing system, as it requires information that can't be addressed in a public forum. I will close this topic and my colleagues will take care of the rest through that system.
Customer Service is located in the upper right corner of the game (browser or mini-client)--see screenshot below.
This will prompt a new window; then select "Recharge Problems" and create your ticket there.
