You need to select a total of 12 ninjas to get those rewards since the game counts "12 ninjas" as "12 ninjas", not "100%". You using/selecting 11 means 100% of surviving units but 1 unit short to 12 ninjas. And you need to select 12 ninjas in all 12 checkpoints too.
