In order to get all 3 stars you need to:

Defeat all enemies on the map - Including the special Challenge (blue circle)
Not have more than 3 deaths
Fulfill the mission goal (i.e. collect an item, defeat the "boss" pack)

Don't you also have to get all the treasure?

Treasures are optional; they aren't required to get 3 stars. However, they are highly recommended to be picked up, as they contain potentially valuable items (Equipment Materials, Character Evolution Items, etc). There is always a chance of obtaining Revival Potions, but each and every Material and Item counts during the building phase :P

no you do not

to beat all enemies you have to kill all the red dot enemies, the blue seal challenge enemies, and finally the boss red dot with a circle around it enemies, if you don't challenge the blue seal you will not get the challenge done

Do you mean character fragments? If so, you have to clear the Elite Instances.

* Clear Stage
** Dont die
*** Clear all enemies
