I also spotted an error and while I dont have a screenshot, the question / answers were :

Hashirama Senju
Itachi Uchiha
Tobirama Senju
Itachi Uchiha

I believe one of the Itachi should be "Madara Uchicha".
I must say that I find the exam to contain A LOT of grammar / bad english / text errors... You should have someone review the english as it makes a text event, almost impossible to comprehend and succeed.
It becomes quite frustrating!
