when you check ninja tab and see your lineup, theres move 1-4. in battle move 1 will ALWAYS go before 2-4, unless your skill is turn based like hidan/sasuke barrier. if you are facing move 1 x move 1 ninja. initiative will play a role in this. otherwise theres no bull* like its random etc. server lag can screw you severaly as it takes a bit of time before game realized you are casting. so its possible with bad connection to be "slower".

    You don't "cast" normal attacks.
    so no, that's not it.
oh you are mistaken. as i stated before. if skill of your enemy is registered before your mystery its very possible for him to be "faster".
