Hello,
From what I have been told this assist card only works when the ninja actually gets hit and suffers damage. If the damage is negated with a shield or through dodge, it should not trigger. Are you still experiencing issues with it even when taking actual damage from attacks? If so, can you record a snippet of a fight for me so I can send it in to be fixed?
