I think like many multiple choice questions we encounter, it can be open to interpretations. However, the expectation is for you to choose the best answer out of the available options =p
I think the question is actually saying, what ninja also goes by this name, "The Copy Ninja". Rather then the question inferring that there is another ninja or character by the name "The Copy Ninja", the question is more asking what ninja is also referred to as "The Copy Ninja" aside from what he/she is known as.

XD good question though LOL and I agree with your point about the Third Hokage. I guess Orochimaru is the "indirect" cause of sadness.
