I've pulled 2, one in the first week of mines (you can bet that even I didn't believe it), and the second maybe 6 or 7 months later. So it happens, just SUPER rare

From my experience, I only got 1 so far since the start of this Cave Mining Feature .. It was during the 2nd year Anniversary of Naruto Online .. Other than that, nothing but i always get Blue and Purple Orbs .. Still thankful for it as it is an Orb and not Ores xD ..

I think the chance is probably ~1/200. I also think it is like your reward for TI, It is decided before you even click on the card. So, don't even worry about which card you click on.

I have, and my server isn't that old (mid-October, if I remember correctly). I've only gotten the one, though. I suspect it might be on a timer like so many other things are (i.e., recruitment), but I don't have any data to support that.

It's very hard to know exactly how it works, but it's not impossible that it is "on a timer" with smaller odds increasing gradually over time...
