No, I usually only have 2-3 Sword-Bearing ninja. And it happens sometimes even from a ninja with a Mystery that doesn't negate Root of Warrior's effect.
Kabuto will sometimes keep doing his action before doing his Mystery like 5-6 times. He's even done his healing standard 2-3 times in a row once before. It only counted as just once, but he did the action 2-3 times.
As for GNW Sai, I must've misread his talents since I don't really use him. I've just always seen him summon 2 when facing him in arena, so I assumed he automatically summoned 2. That's a mistake on my part. Sorry. ^^'

Ultimate Ninja/Survival Trail will adjust Ninja skills, as they increase the difficulty to better give a challenge.
As for the Root of Warrior issue. Please provide a scenario to allow me to test out if there's a bug or not.
Which ninjas were you using?

No, it's not the one with Han.
It's only ever happened twice all my times challenging Survival Trials, and it was when Ultimate Training was brand new. So it may not happen anymore.
To answer your question about Root.
From the ninjas you've named. All of which were Swordsman, it means one of them will not receive the RoW buff.
That could explain the sword-ninja being debuffed.
As for the others, I'm quite confident it's due to ninja mechanics involving UT. -
