The bug the OP mentioned happens when you go into arena start finding a match -> press esc to get out of the arena screen while still finding a match -> attempt to change lineup.
Going to any set after that will clear the lineup and leave only your main. Attempting to add a ninja will result with Request Timeout.
The bug have happened to me twice, neither time did I do what you describe--I never do the arena-esc thing.
That said, I do suspect that it might have something to do with connection, probably a pocket loss somewhere causing the team to load on your end properly, but your end ends up "thinking" it's finished so sends a new line up saved data to the server, overwriting what is already there. Something to that effect anyway. -
