After reaching a certain number of ninjas (not entirely sure exactly how many) instead of having a reward at every ninja, it becomes every 3 ninjas.
It's possible it becomes more sparse later, but I'm on 3s still

Probably your first time reaching that tier?

I highly doubt that's the case. In the mission it said 65/66 Ninjas when I did it. And it even says 66/67 now.

It took me a while, but I finally got a guild mate who happens to have 65 ninjas to check for me
The quest is 65/67
So.... no it didn't.
